Tracking Site Performance Before Sponsors Track It for You

Sponsors and CROs track enrollment against plan. They monitor screen fail rates, query response times, and visit completion rates across all their active sites. If your site is underperforming on any of these metrics, the sponsor typically knows before you do, because they are comparing your numbers to every other site running the same study.

The sites that maintain the best sponsor relationships track these metrics internally and share performance summaries proactively, before the monitoring visit.

Five metrics every site director should review monthly:

1. Enrollment rate vs. target. Actual enrolled vs. projected at this point in the enrollment period. A 20% lag at week four is a staffing or outreach problem that is fixable. A 20% lag at week twelve is a structural problem that needs a sponsor conversation.

2. Screen fail rate. The percentage of screened patients who fail eligibility. A screen fail rate above 60% in a study with straightforward eligibility criteria usually indicates a pre-screening gap. Tracking this alongside the enrollment challenges typical for your indication provides useful context for the trend.

3. Protocol deviation frequency. Categorized by type: procedural, documentation, or patient-facing. Rising deviation counts are an early warning signal for coordinator overload or SOP gaps, not just one-off errors.

4. Data query turnaround. Time from sponsor query to resolution. Most sponsors expect resolution within 48 to 72 hours. Sites that consistently run longer build a perception of disorganization that compounds across the life of the study.

5. Visit completion rate. Scheduled visits completed vs. missed or rescheduled, by study. High miss rates signal patient retention problems or scheduling gaps that need to be addressed before the next monitoring visit.
